Observed evidence records a condition found through an authorized assessment process, together with its source, collection time, method, confidence, freshness, and known limitations. An observation is not automatically a diagnosis or outcome.
A bounded inference is an interpretation supported by identified observations but not directly proven by them. It must remain labeled as inference and retain the limitations that constrain its use.
Organization details, goals, concerns, contacts, and authorizations supplied by a client are labeled as client-submitted. They provide context but do not become independently verified claims merely because they were submitted.
A completed task becomes a verified improvement only when post-change evidence is collected using a stated method and compared with a baseline. Unavailable or inconclusive checks remain labeled that way rather than being replaced with invented values.
